Anybody seen or know about supplement for medical exam

Hi there.

I have received a RFE for medical. It states take this sealed envlope to the doctor and complete section 3 of the supplement for medical exam. Any idea what this is? anybody with any info please reply
 
Your immunization history is recorded in this form by the civil surgeon. When you go for your medical, the civil surgeon fills this sheet (usually the physician has a standard photo copy of this form) based on his evaluation of your immunization history, and attaches it to your I693.

Show your physician your immunization documents. If you don't have any documents, then you would have to do your immunizations. It would be cheaper to have them done either thru your medical insurance, or if they don't cover, at the county clinic.

For chicken pox, past scars, if obvious is acceptable as proof that you've had chicken pox.
 
gcpain1, I also have received RFE on this, my RFE clearly says that I need to take Vercilla Vaccine supp. That was not available when I did it first time.
I went to the Dr. office, Got the Vaccine, got the sealed envelop from them, that's it.
 
Thanks for the replies. I just got back from the hospital. I had to take tetanaus(sp?) shot. Section 3 of medical supplement is a small area where the doctor will have to check a checkbox intdicating that we are ok and are immuined. So i am good to go now. Thanks for all youy help.
