Anybody knows what should I do with I-215W

Hi all;

I applied my citizenship long time ago (10/05). Got internviewed on April 06. Did couple of Finger Print. Recently I received a letter to ask for more information. To be more specific, they want me finish attached I-250W form and give them two new phonos. I have no problem with the photos. But with I-250W, I finished it and have it notarized by an notary. There is one line at the top of I-250W form, I don't know if I put anything there, or the Notary should put anything there. "Before the following officer of the USCIS:_________________".

Anybody had same experience, what should I do with this line?

My guess is that you leave that blank and have USCIS fill it out.
 
Thanks Bob. I was guessing the same. But just want somebody who experienced it or expert's input to confirm this.
 
Mary, did you send out I-215w? Any updates on your case? Are you in San Jose area?

I just got the same letter today too. My application waiting time ｉｓ even longer than yours. I applied June, 05.
