Anybody applied EAD recently?? Where to send the paperwork?

Did any of you applied EAD for your dependent?
if yes, where did you send/filled your paper? I mean which address ??


And appliation is saying that they need cross-face picture, but i heard all they will accept is passport kind of front face picture .. any idea?

The forms are old and still have outdated information.

You need to use front face picture...
