What is form FD-258

Can someone tell me what form is to be filled when I go to FP. I saw
reference to FD-258 at INS website.
Please let me know.. TIA..

painless experience. park in china town (8th exit off 676) and walk south a couple of blocks to service center on right Pick up and fill out the form they provide- name, address, visa stage etc. Make sure you have two ID\'s with you- passport is not necessary. Watch the telly for about an hr until your # is called. They scan your fingers and thumbs individually and then your entire hand-no ink is used. They give you the form you filled out as a receipt and thats it. takes about 1.5 hrs.

how early should I reach there. Is 30 minutes prior to scheduled time
early enough? Can we bring our kids inside INS office?
