what should covering letter look like(format)? what if I was seeing only 2-5 patients per week?
apart from tax returns,W2's,office lease,bills(telephone/heating/rent), do u need to send anyother proof of practice(like patient billing? does it affect patient confidentiality?).
How do u define "proof of practice"?
