After DUI - got first H1B stamp - applying for 2nd H1B stamp

Hello Friends,

PLEASE HELP !!!

I had a DUI on Oct1st 2004 - after that i got my H1B stamp successfully in Feb 2005 in US consulate in Mumbai - now i am going to India for my 2nd time H1B stamp - do i have to mention again in my D156 form about my DUI arrest in Oct 2004 and go through the whole process again??? as i already got it stamped after that and has gone through the whole process...
