after 5 years, what are the opitions?

we have discussed this before, but i'm not clear if anyone came up with a definite answer.

once 5 years are over for an NIW physician and the I140 is approved,what are the options?

change location?
change employer?
change nature of work? (research, consulting, industry, administration etc)

anyone knows?

ps sorry for the typo above... "options"
 
Read the next thread about the GC approval for a former NIWer. I think I interpret it as it is written; that is, one can freely change employers as long as the evidence exists for the 5 year completion.
But there is a twist to the story..... This was a VA facility, and I assume the fellowship was also at the VA.... although the nature of the job has changed, broadly speaking it is still medicine, and by virtue of it being at the VA, is in "National Interest" (as it would to work in a HPSA). So the approval of this case still leaves a question mark- does one need to continue working in an area that would be deemed to be in "National Interest"?
Guys, if you have a similar case, share your experience esp if GC has been approved or denied (i.e after 5 year completion of NIW and changing employers)
