you have two options left,
1)if you are residing in the same city where you are born, go to the brith and death office along with your passport and ssc certificate and ask them to change which they will do in 10 min if you pay them 100 rs. its very easy for you to do
2)in the second scenario where you cant do the first one take a affidavit, and the format is as follows
Affidavit of Birth(This should be on a stamp paper, 10 Rupees stamp paper is fine)
We, the undersigned (1) Mr. Rajeev Thakkar, Hindu by caste, aged 55 years, occupation Agriculture, residing at :-
82, Main Street, Nasik-Pune Road, Nasik 422 011, India
and (2) Mrs. Deepika Thakkar, wife of Mr. Rajeev Thakkar, Hindu by caste, aged 50 years, occupation Household, residing at the same address as above,
do hereby take oath and state on solemn affirmation as under that :
We are husband and wife. We are married at XVillage, Taluka ABC, District MNO, India on 9-4-1962. We are living together as husband and wife. Through our wedded life, we have a daughter named Jyothi Thakkar (married name Priya Thakkar). Our said daughter was born at Nasik on 25-6-1964 (Twenty fifth June Nineteen Sixty Four). The above fact is true and correct and we are the parents of the said Jyothi Thakkar. We know the fact very well. The birth date of our said daughter Jyothi is also mentioned in her college leaving certificate as well as her passport. Since the birth certificate is having name slightly different, we have executed this affidavit to prove the above fact.
The above stated facts are true and correct to the best of our knowledge and belief and we know that to make a false affidavit is an offense.
