Affidavit of Support

You have to use form I-134.If the sponsor is in the United States, the affidavit may be sworn to or affirmed before an immigration officer or before a notary public or other official authorized to administer oaths for general purposes. If the sponsor is outside the United States, the affidavit must be sworn to or affirmed before a U.S. consular or immigration officer.

Samuel5028 how do you know that?
I never read it anywhere... For form i-134 copies are accepcted.
I dunno for form i-864, this one may require authentification. But not the i-134.
 
I-134 form doesn't need to be notarized as Jeffy correctly put it. The Sponsor just should sign it with his full name (not just signature.) I-134 instruction page explains it in detail.
 
Does the I-134 need tax returns with it? If so, how many years and do they need to be originals? And what about an employment letter? Does that need to be the original too?

Supporting documents to be submitted with the I-134 form are:

1) 1040 (income tax return) for past 3 years (or most recent year)
2) W2 (wage and tax statement) from all jobs in the past year
3) Employment letter or pay stubs
4) Bank and asset statement
The more you have, the more you will be comfortable. The most important, however, is the I-134 form itself (one form for every family member applying for DV.)
 
Ziva, hopefully copies of all documents will do. You don't need original (except for the I-134 form itself.)

I dont think you need all this as supporting documents.
The instructions state "as appropriate" and it cites only these:

"A. Statement from an officer of the bank or other financial
institution where you have deposits, giving the
following details regarding your account:

1. Date account opened;
2. Total amount deposited for the past year;
3. Present balance.

B. Statement of your employer on business stationery,
showing:
1. Date and nature of employment;
2. Salary paid;
3. Whether the position is temporary or permanent.

C. If self-employed:
1. Copy of last income tax return filed; or
2. Report of commercial rating concern.

D. List containing serial numbers and denominations of
bonds and name of record owner(s)."

So it's either one of these or more if you can.
