Affidavit of Birth

Hi,
My I140 is pending at VSC and My birth certificate has name in short.
Do I need to go for an additional support document like an Affidavit of Birth? If so, can it be a joint affidavit from my mom/dad or need to be separate?

Also I know guys out here in this discussion board would surely have a sample affidavit form which u can either post out here or send via mail which ever is convenient...
