Affidavit of Birth Certificate

I do have a Birth Certificate but was obtained very recently (an year ago). My attorney says that I need to get an Affidavit from my parents support my DOB/Birth Certificate since the certificate was issued too recently...

Is this really required? If so, could I have a template(content) of the affidavit plz?

I don't know what objection can there be to recent issue of Birth Certificate. You are able to get it any time. In any case BCIS does not accept affidavits- they want municipal certificates only. Typically affidavits have to be obtained from non-parents and also from non-minors who were present at time of birth. If you dig the forum you can find an affidavit format or even in the important link at top of this forum.

The Birth certificate I have recently obtained was indeed from a municipality office...

You need to send an affidavit also in cases where the birth certificate taken recently. Sample is attached.

Point 4) in the attachments says "xxxxxxx’s birth was not registered with the municipal authorities."

But in my case I've registered but only LATE. Do I need to alter the wording to reflect it or leave it as it is???
