Affidavit for Birth Certificate

Hi,
My birth certificate has my mothers name misspelt and my wife's birth certificate has her name misspelt.I was planning to get an affidavit for this and can anyone of you let me know the format for this.Also has anyone else been in a similar situation who completed their CP sucessfully.Can I know how complicated this situation is.
 
What you need...

You need to have a Non-Availability certificate and affidavits from both parents. Since you already have birth certificate why don't you get it corrected? That's a lot simpler...

Send an email to Bombay (or respective) consulate and ask this. They are reply quick.
 
Name wrong in SSC too

Your suggestion seems a valid one.But my problem is I have my name misspelt in my SSC certificate too.And the name is misspelt in the same way in both my Birth and SSC certificates.So I was planning to get a common affidavit saying that I am known by both the names(Something like an alias).There is no way I can get my SSC certificate corrected now.Any suggestions?
