Affidavit for Birth certificate and Marriage for Family Based GC

Hi,
My elder brother, who is a US citizen, wants to sponser my parents for US GC. My parents do not have birth certificate and marriage certificate. We are planning to get an affidavit from a senior citizen who knows both my parents' birth and marriage. Would appreciate if anyone would let me know the format of that affidavit.
