Advanced Parole Extension

I am in a slight predicament.

Mailed I-130, I-485 - November 2006
2/10/07 Interview DONE

Still waiting on Namecheck.

I had applied for Advance parole and used it to travel once. When I reentered, the advance parole was stamped and the expiry date was May 13th 2008.

However, I am planning on travelling again and plan to re-enter by May 3rd.

How do I apply for renewal of Advance Parole, just in case my trip gets delayed? What forms should I use? USCIS website mentions I-512 or I-512L but I can't find those forms?

Can someone advice?
 
There are two dates to look for. Firstly the expiry date on the AP document itself. Secondly, the date on your I-94 that was issued when you first used your AP. You cannot use your AP after the first date has expired. I am assuming you have a multiple entry AP document.
