Additional Evidence? I-130 for mother

Hello,
My birth certificate doesn't show my mother's name; as back in those days it had an option to record father or even grand-father's name only, mine shows my father's name only. I filed I-130 for my mother along with this birth certificate and another official document which listed name of my parents, my name and my siblings. My early health and school records also reflect my father's name only.

USCIS required more evidence to prove mother-child relationship so I've following few questions:
1. Is it a good idea to get my birth certification updated with my mother's name or should I leave it like this to avoid more complications and just submit additional evidence as requested?
2. As I can submit two affidavits from 2 different persons who were know to my family and had knowledge of my birth. Do they av to be 18yrs old at the time of my birth to be considered as qualified to testify?
3. Can affidavits be on a regular print out and signed by the person or has to be on some official stamped paper or notarized paper?
4. If I choose Blood or DNA test how much does it cost?
5. If I choose Blood/DNS test does USCIS requires the test results to further process the case or they process the case regardless and need test result at the time of my mother's visa interview?

DO NOT commence DNA testing ahead of making arrangements by request and/or consent from USCIS and any Consulate involved. You can waste alot of money if you don't comply with procedures properly.

Affiants have to be old enough to remember something first hand.
