AC21 Usage Job Title/Description

GC puli or any AC 21 Gurus,
I have aquestion regarding AC 21 portability. I am an anesthesiologist(Physician) and my labor/i-140 is approved. PD april 18 2004, EB2 (o.dot classification as Anesthesiologist) if I do a year of extra specialisation in Pain management( which is a subspeciality ) then apply for AC 21 and take a job as pain specialist( there is no code for this specialisation under o.dot.net jobs classification ) , is it duable? Any advice will be appreciated.

I am nearing that career change (acads to industry) and thanks to blasted 'name check', which is pending since 1 yr now, may not get GC in hand in time. if anyone has similar experience, please do share.
