AC21 and I 485

Hi -
I filed my I-485 in 2001 July ,Fingerprint done in Dec 2001 and results came in 2002 Jan, and have not received my green card. Repeated enquiries by self and also senator elicit the same reply - routine processing...

Meanwhile I joined another company in April 15 2002 worked there for 3 years and in May 2005 joined another company, both solid companies, ( the first one that I filed was with an Indian run - small consulting company, which has gone under now- whereas these 2 are solid 15000 + employee companies). My salary has gone up by about $30000. I am still in the same job category.

The problem is I never intimated this change of job to INS. The first time I did not know about it - I was told I can change after 6 months of I-485, so I did. Then once I knew about it it was already a year in the new job and I did not know if I should tell this now or not.

Can I file for AC21 at this point?
If so is there any specific form for it?
What are the documents that should accompany my application?
Should I mention both job changes or just the latest one?

Is it complicated and is a lawyer necessary for this? I have been making the EAD applications myself for the past 4 years and is this similar in complexity to that?

-----No form for AC21. new employer permanent job offer with same/similar job duties+copy of I-485 filing reciept+cover letter to invoke AC21 with pending I-485
