A question regarding H1/H4 application

Hi,

I am visiting India next week to get married.I want to bring my would be wife to USA.How do I apply for her visa right now?
Can I fill up my application(DS-156 and 157) forms now,take an appointment date and then add her application later?
Or should I declare myself as married in DS 156 right now since I intend to go to consulate only after marriage?
