90 Day issue

I moved to Hawaii on 7/31/2012 from California. I filed my N-400 on 10/29/2012. I took the test and I passed. But my entire application was withdrawn because of the 90 day issue. Do I have to pay again to put my file in again? I heard that there were at least 2 chances when you fail a test, but I do not know if this applies to being failed.
 
You have to reapply and pay the fee and do fingerprinting and everything else all over again. You get a second chance for that test, but not for other denial reasons.

I heard the fee has two portions-one is for USCIS and the second portion is for FBI crminal backgournd search. Since FBI finding is valid for 18 month one can get second portion wavered
 
No notice of Denial

Also I never have gotten a notice of denial. I called the national customer hotline and they said they were supposed to send a notice of denial if it was filed too early. I never got it either.

What do you mean "your entire application was withdrawn"? You had an interview. Did the interviewer push you to withdraw the application? When was the interview?
