7th year stamping question

I am in 6th year of my H1-B and the same is ending in November 2007 and I recently received my 7th year extension valid till November 2008 (valid from Nov 2007 till Nov 2008) and current visa on my passport was expired in December 2006. Now, I have to travel to India in September 2007 and have following question:

1. Which petition should I use to make visa appointment?
2. Till which period a new visa would be granted (will it be till November 2007 or will it be November 2008?

Please suggest.
