485 Filing question

I got my labour approved and now I am filing 140/485 concurrent. I belive I can send both application together. Now my question is what option I should select on I-485 Part II (Application Type)? I would appreciate any kind of quick response.

---As your I-140 is not approved I hope you have to fill up H, and explain Concurrent I-140 and I-485 filing (I am not sure) Let the Attorney handle the I-485 and I-140 filling

What documents if any do I have to carry for my medical examinations?

1. Proof of imminizations - if u have.
2. valid ID
3. Passport
4. My doctor had the medical forms with him. Check out whether u have to bring them or the doctor has them.

Isnt the passport valid ID by itself, what else could I carry to show identification?
