485 Birth Certificate Doubts..Urgent

I am preparing the 485 documentation.May I know

1) When they say copy of passport documents ..that means both the passports(myself and my wife) is it?

Its not only me..Am I correct?

2) Also for the Birth Certificate if a person's original birth certifcate is avaiable like if it is

issued 2 months after the person is born, for eg, if a person born on aug 1976 and the birth certificate issued
during october 1976 will that be considered as Original?

My another question is on the names on the 10th std and birth affidavits.

My 10th std says only my first Name. For eg.. RAJA

My affidavit written by my father says that my name is RAJA RAO(RAO is my dad's name).
RAO is the name entered in my passport as my DAD's name.

Will this be OK?

OR my birth affidavit should say that RAJA only similar to that 10th std.

---Yes, both your's as well as your wife's passport copies are required

Names on your BC/Passport/SSN/and all INS/USCIS application forms should match, if not you need to take proper action to get it corrected.

My dad's name on the affidavit and the passport(i.e Father's name on passport) should be the same too...
