Yes, you are missing quite a bit:
- Form G-325A (all 4 copies)
- Copy of your I-140 approval notice
- Copies of all pages of your passport, including US visa pages, and
COLOR copy of your biographic/picture page (you can actually enlarge it as well)
- Copy of your current I-94, front and back
- Copies of all I-797 approval notices, as a proof of having maintained legal non-immigrant status

- Writing a cover letter wouldn't hurt either, one listing the contents of the package.
As for I-693, you won't have to fill out anything. Just show up for your appointment with an USCIS licensed civil surgeon and they will do the rest.
