Yes, you are missing quite a bit:

- Form G-325A (all 4 copies)
- Copy of your I-140 approval notice
- Copies of all pages of your passport, including US visa pages, and COLOR copy of your biographic/picture page (you can actually enlarge it as well)
- Copy of your current I-94, front and back
- Copies of all I-797 approval notices, as a proof of having maintained legal non-immigrant status
- If your birth certificate is in language other than English, it needs to be translated, certified by a translator and notarized - for more details, visit http://www.uscis.gov/graphics/fieldoffices/scnational/index.htm
- Writing a cover letter wouldn't hurt either, one listing the contents of the package.

As for I-693, you won't have to fill out anything. Just show up for your appointment with an USCIS licensed civil surgeon and they will do the rest.

Good luck!

Yes. Or you can wait to receive your approval notice first. With all the documents you still need to gather (including medical examination), chances are the approval notice will arrive just in time.

-----------------------I think you need I-134 Notarized (for all dependents)
