I am applying for 3rd EAD Renewal on my own. Few Question in the I 765
application form.


In the coulmn 11th. What is the information for Date Field

11) Have you ever before applied for employement authorization from INS?
(Yes)

Which INS office? Date(s)
(Texas Service Centre) ???????????????????


Is Application fee? $175

What is the Address I should Send i 765 Form to ?

BCIS Texas Service centre,
P.O.Box 851041
Mesquite , TX -75185 - 1041

My EAD is gettting Expired in Feb 12th 2005.
Can i apply in 6 months before?

(Texas Service Centre) ???????????????????

----- if applied last EAD from Texas Service Centre then it is correct and write the date when applied, it may be on on EAD filing reciept or attach the EAD copy they will know
Is Application fee? $175
--- yes, it was $120+ new$ 55= $175
What is the Address I should Send i 765 Form to ?

Dear officer,

Please find the following documents for Employment Authorization Document (EAD):

1. Fees Check of $175
2. Form I-765
3 .Two Photographs in envelope with my Name, DOB and A# written at the back of photographs.
4. Copy of Pending I-485 receipt
5 EAD TOP Mailer( that you got with first EAD, EAD was in paper that has Top mailer)
6. Copy of marriage certificate( if spouse is filing)
7. Color Copy of my Passport first page and color copy of driving licence as secondary ID
8. if you have I-140 approval copy or I-140 filing reciept attach the copy(optional)
9 Copy of I-94
