I am applying for renewal of my EAD. I have question regarding column 11.

Column 11 -
Have you ever before applied for employment authorization from INS?
Date ?

Which date I have to put ( receipt date/notice date/approval date) ?

You should say YES. Thre is another place that asks whether it is a renewal or not: RENEWAL.
DATE: It should be the date you sent in the application, not the RD nor the ND. Besides you incluse a copy of the first EAD. In mine, I did not have the exact date, so I put just the year for both first and second. No problem!
