I don't know what they are because my company applied my EAD. I remember I signed a couple of papers and 2 photos.

Yes, it was VSC.

To LCSilence,
I searched old company email and I found an email from lawyer.
They asked me below,

(1) My current home address
(2) The last date and location you arrived into the US. If you have traveled more recently, we will need a copy
of your new I-94 Card (white card stapled in your passport) and a copy of
the stamped page in your passport indicating the date and location of your
last entry into the US.
(3) The last date and location your wife arrived into the US. Again, if your wife has traveled more recently,
please provide the information requested in #2.
(4) A copy of your wife's current EAD card
(5) Home telephone number
(6) We will also need you and your wife to each send 2 USCIS style photos
(photo specification sheet attached)

Actually we didn't travel out side of US during the one year, so (2) and (3) were N/A.
