230 Form Help

Hi - question 14 asks for spouse name "Maiden or family" - which one?! Maiden name or now family name?!

if the spouse is a woman maiden name, if the spouse is a man then family name..

that is helpful, thank you. Have decided not to submit DS-230 because I am doing AOS - called 3 times to KCC and they said only need to submit DSP-122, I was erring on the side of caution, but too much paperwork and three seperate confirmations swayed me!

I submitted both. Am doing AOS too. Won't hurt
