No evidence requred. You just list your last J visa and date of departure which suppose to be >2 years in the new visa form and sign

what will USCIS accept as evidence that you are not subject to HRR...other than the waiver approval or a favorable advisory opinion...are the IAPs and J-1 stamps and letters from both sponsors, H-1B approval notice enough?
