Sri4gc, instead of asking questions, go ahead and call IRS, they should help you in getting it done. They have a 800 number (they give better service than the INS!!!!)
look into www.yourmaninindia.com They can help you get it done. Yes Birth certificates has to be from the muncipalities. Hospital certificates are not accepted.

It varies from country to country and they are listed in the foreign affairs manual (FAM). Affidavits and Non-availability certificates are applicable for people from India.

it is better to take a appt. w/o appt. they are not allowing people inside. You can do a search and find out info about infopass. I think it is worth the wait...
I think you need to personally go to Bombay to get it stamped. It is illegal to be without proper travel documents, if for some god forbid reason you are being questioned.
nobody knows... it was in RFE status and I was trying to find out what exactly is the RFE. My lawyer did a Level 2 enquiry and week later I got the approval letter.
Based on the zip code, it gives me an appointment at Santa Ana. Do I need to take seperate appointments for my wife, kid and me or one appt. is enough?
