if the application is for you then the address is the physical address of where you live. If you mean the address of your family then I would assume it would be where they live at the moment. I'm sure others would post and correct me if i'm wrong :P

Besides the applied date, passports are accepted only via PP agents unless you have the circumstances in your other thread. Form filled online or on the paper itself is the same, the form online you have to print out afterward and makes it neat.
Cost would be the same + the expedited fee. Normally they don't accept applications for same day service unless you have a life or death emergency in another country or flying within 2 weeks etc.
Also you would have to have an appointment for that service too, need to contact the NPIC for that
