If you really want to move out of your country due to your condition, l will advise you take this job and see it as a step to getting to USA. You never know where the next offer will take you to once you do well on this one. And the important thing is that it will take you out of your country...

This is not a problem, go to the embassy with the laminated card. This is what you did 13 years ago and they understand that we laminate our document for preservation.
If you have them go with them but if you dont you dont need them to start your medicals. There are immunization you once in a life time and some you do after a number of months or years and thats why they need to see your past records if you have any. This is what l believe:)
