Yes they give a form At the time of stamping they provide a form on which you have to give your finger prints,signature and you have an option to put down the address where you want your card to be mailed. I did my stamping about a month back. Hope this helps.

yes Yes you can very much get your passport stamped in a local office which falls under different service center but you GC will come from the Service center from where you filed your GC. I know this becos I have already done that way and waiting for my plastic card.
