The left part of your H-1B approval tells the validity of your I-94. So, even if you surrender your I-94 when you leave the country u will get a new one when you re-enter.

I spoke to American Consulate at Toronto abt new appointment letter issued after 19 Nov. They are not aware of any such rules. However, if you want to call them you can do so at: 416-595-1700. Once you start getting options press 0.
