Canada First he can get his Canada PR done and apply at anyone US consulates in Canada .This will be easier than applying US Visitor visa from India.

Ar-11 AR-11 is needed only when you change address within USA .In your case ,you dont need to do anything .

Usps If you are in USA ,send by Certified mail with return receipt through USPS .
