    I would say: try to find exact dates if you possible can, if not try approximate dates in mm/dd/yyyy format if not than give month and year only. Go back to your airline tickets, bills etc. for posible help in locating your dates. Good luck

    Sorry, one more thing: I am resident in this State for 13 months but due to other things (unemployment, sickness of the family member abroad....) I have changed my driving license to this State just recently, upon coming back from abroad. I guess and hope this shouldn't meter??
