I would say: try to find exact dates if you possible can, if not try approximate dates in mm/dd/yyyy format if not than give month and year only. Go back to your airline tickets, bills etc. for posible help in locating your dates. Good luck

Sorry, one more thing: I am resident in this State for 13 months but due to other things (unemployment, sickness of the family member abroad....) I have changed my driving license to this State just recently, upon coming back from abroad. I guess and hope this shouldn't meter??

I did not change my residency. Just went abroad for almost 6 months due to illness in the family. Thank you very much everyone for such a great support with this vague immigration issues. You know yourself how it feels to be in this situation and not knowing what to do. Thanks again.
