draft a personal letter detailing all the abuse ,breach of contact,forcing you to work outside your H1 provision etc.
get a letter from the nursing home manager that you see patients there on behalf of you employer.Invent a reason!

By doing so your employer is forcing you not to comply with your LCA and breaching the waiver obligations which are clearly site specific.Get a support letter from health department ( not necessary but recommended) and file H transfer to another HPSA for the remaining period.
Your reply is absurd.Entry on AP is legal( inspected entry at the port of entry) and your status is parolee but you can still work with the same H1 employer without EAD for the validity of H1 appoval notice (Cronin Memo) you can also apply for H extension
