Probably re-sending forms according to rules is the best thing you can do. Also you may call KCC, explain the situation and ask if the bar code remains the same (do you have the scan of the original one?). If so, re-sending should work just fine. Raevsky is who'll give you a 100% correct answer.

DS-230 doesn't have such a field. Only DSP-122 does.

So, does a single winner have to put his name to 26-28 of the DS-230 form and then his name and address to 29 although 26-28 completely repeat 1-3? Thanks!
