Get a tourist visa instead if you are not sure you will get the OCI in two months. PIO to OCI conversion is probably more expensive than going from a tourist visa to OCI.
I sent my Indian passport along with my application and received the cancelled booklet along with my U visa and registration card. What they don't want you to send with the application is the original foreign passport.
I don't think the passport is officially considered cancelled until they actually stamp it so. In the UK, the Indian High Commission apparently cuts the corner (no pun intended) from the passport so that it is unusable. It is still the duty of the passport owner to have the Indian passport...
Canadian citizens do not need visas to enter the US. You can present your I-797 H-1B NOA document to the CBP officer along with your Canadian passport and you will be given an I-94 and allowed into the US.
Probably not. There seems to be a wide range of time periods for people to see their applications being acknowledged. Some have waited longer than you have. There is also some anecdotal evidence that people who waited long to receive acknowledgment went from "Under Process" to "Granted" much...
You can send your Indian passports and your son's PIO card for cancellation when you send your US passports to have the U visa affixed. That is acceptable.
Actually, stamping in Canada can be more risky than in India. Also, if you get denied in Canada and asked to apply in India, you cannot return to the US. You will have to arrange to fly to India directly from Canada.
Since he would qualify as the son of an Indian citizen, you will probably need to include a copy of his birth certificate and your Indian passport in addition to anything else that is required.
Send your US passport to them and they will send it back with the OCI card and your cancelled Indian passport (if applicable). I sent the following letter.

There's a huge risk in even trying for a F-1 visa anywhere now since an I-130 has already been filed. That's about as strong of an intent to immigrate as one can demonstrate.

I believe the instructions specifically ask you not to staple photos. Although they don't say why, it probably has something to do with the staples showing up on your scanned photo on the visa and the OCI certificate.
