A friend of mine who landed at the Trivandrum airport, was explicitly asked to show the OCI booklet. I do not know what they would have done if he hadnt carried it with him.

I am sorry, I think I created a slight confusion. Kids cannot get OCI if both parents are Indian passport holders. Otherwise, they can get OCI, as Kabes mentions it.
In your case, yes, you can apply for OCI for your kids along with your application.

As you live in Miami, Florida, Consulate General of India - Houston Texas has jurisdiction over your application. So, you should apply for OCI there.
In the OCI application, there is an entry regarding "place of issue of passport", you can enter US there.

OK, So you dont have to really wait till you get the passport in hand. But, you need to apply for one. Once you apply, and if you can get your passport status on line, the reference number of the passport is your actual passport number. You can plug that number in your OCI online application.
You are eligible in the "5 years since green card" category. You can tell the IO officer to tick that category. Shouldnt be any problem.

I do not think so. For SFO I positively know that the Oasth ceremony is twice a month. I think its the 2nd and 4th Tuesday or some such. You can check the calendar of events in the Nobhill masonic center to obtain the correct dates.
