The appointment is for 2 days. So it is not necessary to pay before appointment. You can go on the first day, do the tests and then pick the payment coupon, then after you finish the first day's tests, go pay (in CBA village Market Branch ONLY) and then bring the payment slip on day 2. Sm1smom, I think they have a point. My interview was 30th September 2013 (the last day for DV 2013) and my visa is valid until 27th February 2014.

My instincts, tells me there could be a possibility that someone use your lost passport to get visa from the embassy (could be either IV or NIV but high chance NIV) and so when they check their records they see some questionable issues that could be indicators of fraud and/or criminal activity...

If you put KSCE cerificate, there is no need to worry. It is okay. Eventually the embassy will verify the form. Unless I did not get your question clearly.
