As for my (and my wife's) case, the only exception was Chickenpox in that clinic. Oral statement of Chickenpox history worked. I told them "I got infected with Chickenpox when I was X years old" and the physician allowed no additional vaccine.
At a designated clinic, they will do 1) X-ray 2)Syphilis Serology Test (HIV test is not required after Jan 2010), 3) physical exam 4)vaccination.
In some clinics, they would say they need to measure antibody titer before decision of vaccination (for which they don't count on history/record), but it takes more than a few days. I chose a clinic where I am not asked to take this test.
