Applying for I765 need some serious suggestions

I had applied for asylum in September 2013 and its already 150 days now. I haven't heard anything about the interview dates and no initial decisions have been taken on my case. Before I applied for asylum, I was a F-1 student and when I was in F-1 status I had applied for I765(OPT). However that I765 in F-1 was denied.

Now I am applying for work permit(I765) and I am in pending status. While filling the I765, on number 11, the question reads "Have you ever applied for employment authorization from USCIS"? Should I check "YES" or "NO"? Since I haven't applied for I765 under pending status but applied for when I was in F-1(denied in F-1). If I put yes, then its again asking about "Results(Granted or Denied - attach all documentation)". Which one should I attach ?

Also, what are the documents that I need to submit with the I765 ? and where should I send it to ? I received my first initial letter(not the interview date) from SFO which says I am in Pending status.

Thank you for your reply. I don't have the proof that the previous I765 was denied. What are the documents that I need to submit with this I765?
 
They will find out that eventually so you don't eventually.
So, you didn't got any refusal letter or something for your previous I765?
You can at least include the receipt of I765 which you got when you applied as a student

thank you for your quick response. I am worried on what other supportive documents that i should put with this I765(like Passport pictures and all that). I don't want to get denied this time.
 
You need two passport size pics, I765, Fingerprint notice/Interview notice(if interviewed), Previous I765 letter/notice.
I also included my passport front page photocopy, visa and i94 copy.
