Instead of explaining, i will just write the example:
#1. My wife's name before marriage in her education records: Sunita Kumari (last name missing).
In Indian passport : Sunita Mathur (name changed, removed Kumari).
She never used Sinita Kumari name after the marriage. All travel to us/visa are in the name of Sunita Mathur. Her education records have her old name. She does not have birth certificate.
Should she write:
C. If you have used other names, provide them below:
Family Name: leave blank or write NO LAST NAME or sonething else
First Name: Sunita
Middle name: Kumari
or suggestions?
#2. My daughter's last name was missing from her Indian passport when she first came with us to US.
After coming to US, I got her last name included in Indian Passport and I have to give advt. in Indian and US news paper to declare the name change. It was the the requirement of Indian Passport issuing authority. Now she has correct name as Sajal Mathur
Should she write:

Family Name: leave blank
First Name: Sajal
Middle name: leave blank
or suggestions?
#3. I use my initials at my office. Though in all office records, I have my full name but in day to day communication I use OP Mathur, like time off or emails etc.

First Name: OP
Middle name:
or suggestions?
Some more small confusions:
#4. while writing the A# should I start A123-456-789 in the box in N-400 or just 123-456-789.
#5. The dates in 5 year home addresses : should be mm/dd/yyyy or mm/yyyy?
