I am an Indian who came to US chicago in L1B Visa.
My passport and L1B visa stamp has the last name as my x-husband name.
Thus to avoid confusion, I filled my x-husband name as lastname in the I-94 form itself and I had no problem in the port of entry to enter USA.

I have the court stamped divorce order in hand.

I am yet to apply my SSN at US.

I DONOT want to use my x-husband name any more as my lastname.

1) Is it possible for me to change my last name in passport as my fathers name from US chicago Indian embassy

2) Then have the change reflected in the I-94 ?

Will it affect my visa status if I change my last name now? Since I just landed in US a week back please advice on how to proceeed.

Please advice in detail how to do this ?

----------- Yes, you can.

2) Then have the change reflected in the I-94 ?
------------ I dont think there is a need to change I-94

------------- you landed in US based on your L-1B and not at the mercy of your husband, so that should not cause you any problems

Is it required to change I-94 for SSN

I have got my passport name change done in chicago Indian embassy. :)

I am yet to apply apply SSN. Do I need to change the last name in I-94 form before applying SSN?

There isn't perhaps any legal provision to change name on I-94; neither it is necessary to do so.

For applying SSN, you need to mention the last name as it exists at the time of appying for SSN. Along with the I-94 card, you can show the new last name on your passport and divorce certificate.
