G325-A confused

Hi all,
I am filling out G325-A and I am a bit confused. I am the petitioner, do I need to put JUST my name and signature (as the instructions to the form suggest) and a full completed form for my relative?
Also, I don't have a middle name - shall I put none or N/A?
Please advise.

Total of 8 pages that needs to be filled out by both you (petitioner) and for your dependent (applicant).
4 pages by you and 4 pages by the applicant.
Leave the middle name blank or simply -
