Couple of questions help in filling is 485 /i-130-thanks

Me and my wife are trying to fill out all the forms ourselves just had couple of quick questions

in the -i-130 form question 22, where they ask

22. Complete the information below if your relative is in the United States and will apply for adjustment of status.

what do i write here? the office i am applying to ? or my local state office i am delaware?
 
also does the affidavit of support needs to be NOTARIZED? i read few places that i dont need to do it and even in the i-834 documents it's not mentioned anywhere
 
the new affidavit of support does not need to be notarized; i had sent an old copy myself, which required notarization, and was requested by USCIS for a new copy that didn't require one.
