Question on PIO Application Form for minor

I am filling PIO Application form for my 5 year old son and have the following questions and would really appreciate your suggestions:

1. Should I paste Passport Size Photo on both the sets (DUPLICATE form also)?.

2. What should I do for "Please sign in the box below" for a minor?

3. What should I write for Sec 13. "Date of Expiry" for our passport details as we got our Indian passports canceled after Acquiring US Citizenship & I don't remember the date the Indian Passport was canceled.

On my sons PIO application, I stuck a photo each on both the applications and sent 4 additional photos.

2. What should I do for "Please sign in the box below" for a minor?
On the signature line, I signed as the parent of the minor and in the box below, I imprinted my sons thumb prints.

3. What should I write for Sec 13. "Date of Expiry" for our passport details as we got our Indian passports canceled after Acquiring US Citizenship & I don't remember the date the Indian Passport was canceled.
Sec 13 in the form download from the Embassy of India, Washington DC only asks for the following:
- Passport Number:
- Date of Issue:
- Place of Issue:

I hold an Indian passport and accordingly furnished the details.
