Your NIW petition needs the IGA letter. Without it the chances of an approval are slim.
Each state makes its own little rules on this matter. The states that make life difficult.................well they will get what they deserve i.e a physician shortage which is already emerging nationally.
Michigan will write you the required letter, but only if you are employed. They will not do so if you wish to open up shop in a HPSA. Other than this ridiculous self imposed rule they are OK to work with and will get you the letter in a matter of a couple of weeks.
No Idea. However the best way to find out is to call that states Dep't of health and ask to speak with the person who deals with this issue (usually there is someone assigned to the Conrad 30 program). I am sure they will give your their state specific requirements. Southern states have been quite bad about this issue in the past but they are also some of the states facing the largest shortages now..........Hmm ironic isn't it!
If you have a helpful employer then they can make that call as well.
