Question about I-693

I am about to mail out combined I-130 / I-485 for my family members and not knowing that the doctor's office has Carbon Copy versions of the I-693 available in-house, had taken printouts of I-693 with basic name/address details for the medical appointment.

Instead of using their own Carbon Copy Forms or filling out all 4 copies of the forms I took in, the doctor's office has used only Page 1 of the I-693 forms I took in - (marked below as "Original USCIS A-File") - to complete the medical results and has sealed 3 photocopies of this page in the envelope for USCIS.

Since each of the 4 pages of the I-693 is specifically marked below to (1) Original USCIS A-File, (2) Civil Surgeon. (3) Applicant and (4) Physician or Health Department, will it create any problems if only copies of the first page have been used to complete the report?

I dont think it causes any problems. Something similar happened to me. So I think these surgeons get instuction from CIS, so they know what they are doing. The main thing is not to have the diseases they are screen for.
 
Thanks very much for your input. The medicals are all clear (the doctor provided us with a copy of the completed I-693) so I'm going to send the packet off and hope the forms are accepted as is.

Hey if they are going to need to work, make sure you also include their EAD applications with it. That way they can be authorised within 90 days.
 
Some times Surgeons have no idea what they are doing...

I am guessing he filled the form by hand?

He should have filled it electronically (on the computer, using adobe acrobat reader) and it would fill all 4 pages automatically (as he fills in the 1st page with the info) and then just print it and give it to you.

But oh well, hopefully it would not present a problem. But it could. if you have done the same thing with your G-325As I would think there would be a problem.

But isn't it true that the CIS people dont open the medical forms until the interview? Thats what I heard. So if thats the case, I don't think it would present problems.
 
Thank you all for your input and advice. EAD Applications are also being sent in the same packet.

Re. the I-693 - Yes you are correct they filled one copy of the form by hand and made copies to enclose in the sealed envelope and one copy for us. It would be much better if they did this on the computer and filled out all 4 copies simultaneously as they should be.

I completed all other paperwork myself including the G-325As so I'm sending the packet off and hoping it all goes through without a problem. Thanks again
