It seems it has to be the U.S. address/telephone number.
I had used:
item#15 - indian address & telephone number
item#16 - U.S.phone number
That got rejected.
So, as given at:
http://travel.state.gov/revals.html, I'm resending with U.S.address/phone number.
Also, in the 221(g), the box that is checked is:
"You did not enclose a DS-156, Nonimmigrant Visa Application. You must use the Electronic Visa Application Form DS-156, which may be found at:
http://travel.state.gov, AND PRINT THE THIRD PAGE BARCODE INFORMATION. State your U.S. address in item15 of the DS-156 and your U.S.home telephone number and U.S.business phone number(if you are the principal alien) in item16.".
I had used electronic form with barcode page.
Of above, they had highlighted this statement:
"State your U.S. address in item15 of the DS-156 and your U.S.home telephone number and U.S.business phone number(if you are the principal alien) in item16.".
So, it is obvious U.S. address/phone had to be mentioned, and I did fill wrong info.

I hope APPAN doesn't get into trouble just because of this, as its pretty silly we fill the U.S. address anyway in "address where you will stay". Last time I had filled home-country-address/phone there and it got approved.
